Implant placement in atrophic edentulous jaws can pose a significant treatment challenge due to the extreme decrease in supporting bone structure and the progressive nature of the process of resorption. Patients with completely edentulous maxilla and mandible with implant becomes difficult because of less bone quality, maxillary sinus in posterior maxilla and mental foramen and mandibular canal in mandible. A variety of different treatment strategies have been presented for use in those patients seeking a fixed implant-based solution. There is no consensus as to which treatment modality is superior to the other in literature. The decision depends upon patient factors and the expertise and skill of the clinician. Over the last decade, an immediate single stage treatment with low morbidity has seen a significant increase in ‘graftless’ procedures. In this lecture, we intend to display the one of graftless solution procedures, tilted implants with concepts, such as all-on four, fast and fixed so on. The effects of the placement technique, such as implant tilting, remain unclear, although distal implants are frequently inclined to support a fixed full-arch prosthesis because of the anatomic limitations of the residual alveolar bone. The detailed clinical and para-clinical testing of patients, planning in advance of surgical and denture treatment, following of the surgical the pros and cons of immediate implantation and loading will be discussed, as well as risks and pitfalls.
